Outcomes Platform for Caseload Management and Data Collection

The Know Your Own Health digital platform enables individual services or entire systems to:

  • Evaluate impact

  • Report consistently across cohorts or populations

  • Gain insight for service optimisation

  • Conduct cost–benefit analyses and demonstrate value

Since 2013, the platform has been used to collect outcomes data for individual NHS-commissioned services. It also provided the dataset for the evaluation published in BMC Public Health in collaboration with Imperial College London in July 2025: Health coaching for people with long term conditions and multimorbidity: a mixed methods prospective service evaluation of Structured Agenda-free Coaching Conversations (StACC) in UK primary care

A purpose-built digital platform designed to evaluate the impact of coaching interventions for individuals and populations

What makes the KYOH platform different from other online systems, or coaching templates?

The KYOH platform:

  • Functions primarily as a caseload management system for coaches.

  • Does not impose any specific template, allowing it to accommodate any coaching approach.

  • Supports the creation of a taxonomy of coaching approaches.

  • Is purpose-built to collect consistent, reliable, and analysable outcomes data by providing a clear but flexible pathway for coaches that captures the entire patient journey, regardless of the coaching approach or individual outcome of the journey.

  • Collects and collates a wide range of outcomes data, including verified ‘completer’ datasets that can be linked to service usage.

  • Includes checks and balances to ensure all patients are fully tracked through the coaching journey.

  • Enables system-wide benchmarking, intervention comparison, and meaningful cost–benefit analysis.
